Participant Agreement and Waiver
By checking the box, I acknowledge that I have read, understood, and agree to the terms outlined in this Participant Agreement and Waiver. I confirm that my player(s) are physically and mentally fit to participate in all basketball training activities and have been examined by a licensed medical physician within the past year. I understand that Joe Richardson, Eric Prescott, their employees, representatives, independent contractors, and any property owners or officials associated with the training sessions are not responsible for any accidents, illnesses, or injuries, including medical or dental expenses, arising from participation in this program. I hereby release them from any claims, costs, liabilities, or injuries incurred during training.
I also agree to assume full responsibility for any medical bills resulting from my player’s participation and authorize Joe and Eric to use their discretion in seeking medical treatment in the event of an emergency. I further acknowledge and accept that there is a no-refund policy.
